How to register
We have an open list and welcome requests for registration from patients living in or moving to the practice area. We welcome registrations from students from the University of the West of England campus.
You can register by bringing along your NHS medical card or completing a GMS Registration form available from reception or can be downloaded from this website - go to Other Information and click on Library. All Patients over 45 will be invited for a Blood Pressure check appointment.
GP Appointments system
We run all our GP surgeries by an appointment only system. Appointments can be booked by telephoning the surgery on the day you wish to be seen or alternatively you can pre book up to 4 weeks in advance - you can also book your appointment on line. When you wish to see a doctor you should contact the surgery to make an appointment. Please give the receptionist a brief description of your symptoms, this will ensure you are given the most appropriate appointment. You may not need to see a GP.
Pre-Booking of appointments:
We release the majority of all available GP appointments for pre-booking up to four weeks in advance as we realise that often patients want to pre-book an appointment. The number of appointments available for pre-booking varies from week to week depending on the number of clinicians available. Please note that we cannot guarantee that a pre-bookable appointment will be available with a specific doctor.
We try to make sure you can see the doctor of your choice but if they are not available we may ask you to see another doctor.
Please note that between 8.30-10.00am the 8 telephone lines into the practice are extremely busy. Alternatively you can make your appointment on-line - please register at reception for this service.For general queries please call us after 10.00am when our lines are quieter.
If you are too ill to come to the surgery and you require a visit at home, please call the surgery before 10am. Home visits take up much more of a doctor’s time than a consultation in the surgery, so if you are mobile please come into the surgery to be seen.
Reminders
If you would like a reminder of your appointment sent to your mobile phone please register at reception and provide us with an up to date mobile phone. NB WE NEVER SEND A TEXT TELLING YOU TO RING A PREMIUM RATE PHONE NUMBER - SO PLEASE IGNORE IF YOU RECEIVE ONE AS THIS IS LIKELY TO BE A SCAM
Nurse Specialist Appointments
We have a team of Nurse Specialists who have been trained in the management of a range of minor illness. There are normally two Nurse Specialists working each day. These appointments are available to book on the day and if appropriate can be pre-booked. Nurse Specialists are able to prescribe certain medication and give advice.
NB -Some conditions can be dealt with over the telephone and you may be offered a telephone consultation.

- Sore throat
- Earache
- Conjunctivitis
- Sinusitis
- Urine infection
- thrush
- Emergency contraception - morning after pill
- Constipation } Health Visitor run a drop in clinic at
- Worms } Little Stoke Community Centre each wednesday
- Gastroenteritis }morning 9.00-11.00
- Diarrhoea & Vomiting - by telephone call only
- Rash
- Skin infections - including wound infections and infected toenails
- Nappy Rash
- Asthma - if not ill or breathless
- Cough
- Vaginal discharge
- Minor injuries/joint pains/backache - if not severe
- Hayfever - You do not need an appointment for a prescription simply email your request using our website or complete a prescription request form at Reception
PLEASE NOTE THAT TREATMENT ROOM AND BLOOD APPOINTMENTS ARE NOT AVAILABLE TO BOOK ON THE DAY AND MUST BE PRE-BOOKED. THEY CAN ALSO BE BOOKED ON LINE.

| Travel |
We run a travel clinic for all vaccinations and advice for foreign travel. Please plan early and complete the travel questionnaire as soon as possible as some treatments take some time to give you full protection. NB Not all vaccinations are free and the nurse will advise you of the total cost PRIOR to treatment. This service is also available to non-registered patients |
| Smears | All women from 25-65 are encouraged to have a smear test. Between 25-50 you should have a test every 3 years. After 50 every 5 years. |
| Vaccinations | Routine vaccinations for all ages are available during surgery times by appointment with our nurses. We run flu vaccination clinics throughout the winter. |

Repeat prescriptions
We have dedicated Prescription Administrators to deal with requests for repeat medication. If you have a query she can be contacted on ext 224.
Prescription requests can be requested through this website - please go to our services within the main menu and click repeat prescriptions.
We do not accept requests over the phone. Alternatively, you can drop your request in the prescription box in the foyer- remember to tick the items you require. Your prescription will be ready for collection 2 working days later. If you prefer, we can post it back to you (you will need to provide a stamped, addressed envelope).
Should you wish to ask a local Pharmacy to manage your repeat prescription requests for you, please speak directly to the Pharmacy of your choice. Some local Pharmacies may also deliver to your home. Please allow an additional 24 hours for collection at the Pharmacy.
We are keen to ensure that patients with ongoing medical problems are monitored regularly. If the date for your next review with a doctor or nurse has passed you will be asked to make an appointment.
Please note that the local pharmacies can arrange collection of your repeat precriptions on your behalf. Please contact them directly for information regarding this free service.
Sick certificates
You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness.
Test results
Since our telephones are very busy in the early morning, we ask you to telephone for test results (eg blood, urine, x-ray) between 10am - 4.00pm each day. You do not need to contact the surgery for results unless the doctor advises you to do so. If a result needs further action we will contact you.
To ensure confidentiality, we only release results to the patient, unless alternative arrangements have been agreed in writing.
Non-NHS services
The NHS does not pay for some of the services we provide. These include private sick notes, insurance forms, holiday cancellation forms, medical reports, fitness to travel certificates, private prescriptions, passport signing and some vaccination services. Our fees for these services, which are in line with BMA approved national guidelines, are displayed in the surgery. Please note that our NHS work does take priority so there may be a delay in completing these forms.

Pre-Employment screening

We can offer more extensive executive pre-employment screening to reflect your investment in your senior staff and their value to your company.
Long-Term Sickness Reviews
One of the significant causes of absenteeism is the small number of staff who go on long-term sick leave. We are experienced at dealing with these cases, and we are able to produce valuable information in relation to prognosis and return-to-work schedules.
Health Screening
Health screening can:
- detect serious diseases early
- provide health education leading to improvements in lifestyle and work practice
- provide baseline results for future reference.
Providing health screening keeps your workforce healthy, shows duty of care and makes your staff feel valued. It can be a valuable tool in staff recruitment and retention.
Night Worker Health Assessments
It is now a legal obligation to offer night workers an annual health screen. However, the good news is that we can offer these to client companies for a nominal fee, or a full physical can be undertaken if required.
Drug and Alcohol Testing
In safety-sensitive industries it may be necessary to prove your workforce is free of drugs and alcohol. We give expert advice in relation to this, and provide random or general screening using accepted protocols.
This can be difficult ground- if drug/alcohol testing were in conjunction with a disciplinary policy, this would not be the responsibility of the OH physician- this is “policing” and is the responsibility of the company as part of its security policy. If it is used as a detection for drug abuse, with the idea of remediable help, then OH has a role, but disciplinary measures may still be needed.
Specialist Health Checks
The Stokes Medical centre can carry out the full range of specialist and industry specific tests and inoculations, from trace element testing, lung function testing and audiometry to vaccination for hepatitis or checking immunity levels.
Flu vaccinations
Flu vaccination can reduce sick leave; a study in the prestigious New England Journal showed “Vaccination reduced upper respiratory illness episodes by 24%, sick leave by 43% and physicians' office visits by 44%” (Nichol, Lind, Margolis, et al, New England Journal of Medicine 1995 333:889-993). Need we say more?
Mandatory medical provision
Some medical provision is mandatory, e.g. under The Working Time Directive, night-workers must be offered an annual medical, as must workers in various safety-sensitive industries, such as airlines. We can advise you on this.
